CIRCLETOP FARM KENNEL CONTRACT

Circletop Farm agrees to exercise due and reasonable care and to keep kennel premises sanitary and property enclosed.  The pet is to be fed properly and regularly, and to be housed in clean, adequate quarters.

All pets are boarded or trained, or otherwise cared for by Circletop farm without liability on our part for loss or damage from injury, disease, death, running away, theft, fire, injury to persons, other pets, or other unavoidable causes, due diligence and care having been exercised.  Owner of pet agrees to incur costs to replace any real property damaged by their pet as a result of their confinement at Circletop Farm, which includes but is not limited to walls, gates, bedding, bowls, equipment, and fencing.

If a pet, which is presented for boarding, is found to have parasites, fleas, ticks, worms, or fungus, it will be treated and owner agrees to pay the costs associated with treatment.

During your pet’s stay at Circletop Farm if veterinary care is deemed necessary, an attempt to contact the owner will be made, however, if Circletop Farm deems the state of the pet’s health demands quick action, I shall have the right to seek the care of a veterinarian, administer medicine or give other advisable attention, within my discretion and judgment, and such expenses, being reasonable in amount, shall become the responsibility of the owner of the pet, and due upon discharge.

The pet is not to be taken off the premises without the consent of the owner, unless seeking medical care.

If any charges for boarding, grooming, training, medicine, or veterinary services are not paid within ten days of discharge, or if the pet is not retrieved within ten days after the last indicated time of pickup, the pet is deemed “abandoned” according to Kentucky Statute. KRS.257.100 (3a) Any animal placed in the custody of a licensed veterinarian for treatment, boarding, or other care, which shall be unclaimed by its owner or his agent for a period of more than ten (10) days after written notice by certified mail, return receipt requested, is given the owner or his agent at his last known address, shall be deemed to be abandoned and may be turned over to the nearest humane society or animal shelter or disposed of as the custodian may deem proper. Notice in writing of delinquent accounts shall be mailed by registered mail to the owner of the pet at the address given hereon, and no further notice shall be deemed necessary.  Unclaimed animals will be placed in adoption status, and any fees Circletop Farms receives, becomes the property of Circletop Farm.  Any deficiency is deemed to be delinquent and immediately due by said owner.
I have read and hereby agree to the foregoing as owner of the pet and indicate such with the submission of my reservation to Circletop Farm.
